AIM: Bacillus subtilis var. natto is used in the production of natto, a typical Japanese fermented soybean food. Although the probiotic attributes and health-related effects of B. subtilis var. natto have been reported, the effect on longevity remains unknown. In the present study, the effects of B. subtilis var. natto strains on lifespan extension and the molecular mechanisms governing the prolongevity were examined using Caenorhabditis elegans as a model animal. METHODS AND RESULTS: Synchronized 3-day-old (young adult) worms were fed Escherichia coli OP50 (control) or a subcloned isolate of B. subtilis var. natto Miyagino strain (MI-OMU01) and subjected to lifespan, survival against pathogens and abiotic stress resistance assays. Notably, the lifespan of worms fed MI-OMU01 was significantly longer than that of the animals fed OP50. Moreover, MI-OMU01 increased the resistance of C. elegans to several stressors, including UV irradiation, H2O2, and Cu2+. CONCLUSIONS: Genetic and gene expression analyses using mutant animals suggested that MI-OMU01 extended the lifespan of worms in TIR-1/SARM, p38 MAPK, and insulin/IGF-1 signaling pathway-dependent manners.

We investigated polymer nanofilm (PNF) for use in high-throughput screening (HTS) to promote the development of transdermal therapeutic systems (TTS). The drug permeability of PNF with a 1 : 1 weight mix ratio of poly(L-lactic acid) (PLLA) and poly(methylhydrosiloxane) (PMHS) (PLLA/PMHS (1/1) PNF) and Strat-M® of the transdermal diffusion test membrane, was evaluated using 12 kinds of drugs with the logarithmic value of n-octanol/water partition coefficients of -4.70 to 3.86. The lag time of PLLA/PMHS (1/1) PNF made via polymer alloying was significantly shorter than that of Strat-M® for 10 drug types, and the formation of a highly diffusible PMHS-rich phase accompanying the formation of a sea-island structure was suggested as a contributing factor. Additionally, a high correlation was confirmed between the measured value for the logarithm of the apparent permeability coefficient of PLLA/PMHS (1/1) PNF and the literature values for the logarithm of the apparent permeability coefficient of human skin (r = 0.929). This study shows that PLLA/PMHS (1/1) PNF can reliably predict drug permeability in human skin and can potentially be used in HTS for developing TTS.

AIM: This study developed a risk scoring tool and examined its applicability using data from the Kihon Checklist cohort dataset for 19 months to predict the transition from no certification for long-term care to long-term care level 3 or above. METHODS: Data were collected from 26 357 functionally independent, community-dwelling older adults in a Japanese city who answered the Checklist in 2014 and were followed for 19 months. Individuals certified for long-term care during the follow-up period were classified into three levels depending on their certification status: low, moderate, and high long-term care levels. Relationships between the Kihon Checklist domains and high long-term care levels were examined using the logistic regression model. A score chart predicting incidents of high long-term care levels was created to facilitate its applicability. RESULTS: As of 2016, 971 participants were certified for long-term care (3.7%), of which 168 (0.6%), 357 (1.4%), and 446 (1.7%) were certified as high, moderate, and low long-term care levels, respectively. Variables associated with the certification of high long-term care level included difficulties in activities of daily living, a decline in locomotor and cognitive function in the Kihon Checklist domains, and age. The score chart was created based on these variables and demonstrated excellent discriminatory ability, with an area under curve of 0.817 (95% confidence interval: 0.785-0.849). CONCLUSIONS: The Kihon Checklist can predict the future development of a high degree of dependency. The score chart we developed can be easily implemented to identify older adults at high risk with reasonable accuracy. Geriatr Gerontol Int 2022; 22: 797-802.

Context: The average Japanese lifespan became the longest in the world in 1986. What factors give the Japanese this longevity? Washoku, or the traditional Japanese diet, is respected globally for its nutritionally-balanced and healthy eating habits. This uniquely Japanese way of eating may be one factor that helps extend the Japanese lifespan. Objective: To explain the nutrition intake characteristics of today's Japanese elderly compared with their children's generation and to discuss the association between nutrition intake and various diseases and health issues in the general adult population. Methods: This study compared the characteristics of nutrition status and nutrition intake among today's elderly and their children's generation by using National Health and Nutrition Survey scores. Results: Japanese elderly had high adequacy in all nutrients as well as a high intake of potatoes, pulses, vegetables, fruits, algae, and fish and shellfish compared with their children's generation. Conclusion: Nutrition intake among the Japanese elderly had the characteristics of washoku, but these characteristics were not passed on to the next generation. Extension of the average lifespan and improved health could be achieved by modifying nutrition intake, even after reaching the age of onset of lifestyle- and age-related diseases, typically the 50s.

Background: Increasing dietary fibre (DF) intake through a habitual diet is recommended for preventing diabetes. Aim: To investigate a stepwise approach to nutrition education on DF intake among young adults in Japan with the largest deficit in habitual DF intake from the recommended value. Subjects and methods: Plasma glucose levels were measured in 54 adults in their 20s and 30s (29 men, 25 women) during fasting and at 30, 60, 90, and 120 minutes. Habitual DF intake and postprandial plasma glucose level were analysed. Results: DF intake was low (men = 5.7 ± 1.5 g/1000 kcal; women = 6.3 ± 1.2 g/1000 kcal) with no sex difference. Comparison between low- and high-DF groups based on the cut-off point (7.0 g/1000 kcal) showed that the pattern of changes in postprandial plasma glucose levels was significant, and a significant increase was observed at 30 minutes in the low-DF group. Comparison by food group demonstrated that habitual intake of potatoes, green/yellow vegetables, other vegetables, seaweed, peas and beans, and fruits was significantly higher in the high-DF group. Conclusion: This study provides valuable information regarding food selection for preventing diabetes and suggests that 7.0 g/1000 kcal of DF is an effective target value for a stepwise approach to nutrition education in Japan.

OBJECTIVE: To examine the effect of an adapted simulated patient (SP) intervention on self-efficacy in nutrition care process skills. DESIGN: A repeated-measures design using a 25-item survey divided into 7 nutrition professional practice competencies (PPCs) employing a 5-point self-efficacy scale (1 = lowest to 5 = highest) administered immediately before and after the intervention. SETTING: A private Japanese university. PARTICIPANTS: Ninety Japanese third-year dietetics undergraduates aged 20-38 years. INTERVENTION: An adapted SP activity practicing nutrition care process skills for the infirm elderly population. MAIN OUTCOME MEASURES: Pre- to postintervention self-efficacy response scores and feedback. ANALYSIS: Mean preintervention survey scores were used to divide participants into statistical quartiles (Q1 indicated lowest mean scores and Q3, highest mean scores). Wilcoxon signed-rank tests compared each PPC's pre- and postintervention means. Kruskal-Wallis tests examined changes in quartiles' scores within each PPC. RESULTS: Self-efficacy improved significantly in PPCs relating to application of appropriate medical ethics and interpersonal skills (P = .02), appropriate nutrition assessment (P = .04), and creation of a nutrition management plan and nutrition intervention (P = .03). Self-efficacy of Q1 and Q2 rose significantly in most PPCs, although not for acting as a dietitian within a medical care team, whereas that of Q3 decreased for all PPCs. CONCLUSIONS AND IMPLICATIONS: Among initially low self-efficacy dietetics undergraduates, the SP intervention enhanced self-efficacy in 3 of the 6 PPCs practiced directly and may facilitate more realistic self-views among initially high self-efficacy students. However, further research in the design, implementation, and efficacy of this type of training is recommended to gauge its effects on the quality of related professional practice.

The occurrence of preeclampsia before 20 weeks of gestation is rare and usually associated with trophoblastic diseases or antiphospholipid syndrome. Here, we report a case of preeclampsia before 20 weeks of gestation in the absence of the aforementioned disorders. A healthy 30-year-old nulliparous woman presented with new onset of hypertension and proteinuria at 18 weeks of gestation. Fetal ultrasound did not reveal any abnormalities. Empirical steroid treatment was initiated based on a tentative diagnosis of underlying renal disease. The clinical course of the disease was progressive despite steroid treatment and the fetus died in utero 8 days after the initiation of treatment. Following delivery, a renal biopsy was performed and provided a diagnosis of preeclampsia. All symptoms resolved postpartum. This report demonstrates that preeclampsia may occur before 20 weeks of gestation and should always be considered in the differential diagnosis of pregnant women with new onset of hypertension with proteinuria. Previous published cases are summarized briefly.

PATIENT: Female, 28 FINAL DIAGNOSIS: Single intrauterine fetal death Symptoms: - MEDICATION: - Clinical Procedure: - Specialty: Obstetrics and Gynecology. OBJECTIVE: Rare disease. BACKGROUND: Single fetal death in monochorionic twin pregnancy may result in poor perinatal outcome of the surviving twin, including neurologic sequelae, other organ injury, and death. In most reported cases of poor perinatal outcome in the surviving twin, monochorionic co-twin death occured after more than 20 weeks of gestation, while few with earlier occurrence have been presented. CASE REPORT: A 28-year-old primigravid woman was referred to our hospital at 18 3/7 weeks of gestation for perinatal management of single fetal death in a monochorionic-diamniotic twin pregnancy. Our first evaluation by ultrasonography revealed a dead twin sized at 16 weeks of gestation, and an alive one with normal size and appearance, together with 1 placenta and 2 amniotic cavities with normal fluid amounts. At 20 3/7 weeks of gestation, ultrasonography showed that the surviving twin had bilateral ventriculomegaly and dilatation all around the subarachnoid cavity despite a normal head size. Fetal magnetic resonance imaging revealed remarkable atrophy of the cerebral cortex. After counseling, the patient and family chose termination of pregnancy, and artificial abortion was performed at 21 weeks of gestation. The aborted fetuses were not anomalous. Autopsy pathological findings confirmed encephalomalacia in the surviving twin. CONCLUSIONS: Recent development of imaging device make it possible that several abnormalities in central nervous system can be detected in detail at earlier gestational age. It is important to keep this condition in mind even though single fetal death occurred at early gestational age.

We have recently identified tumor necrosis factor (TNF)-α-induced phosphorylation of epidermal growth factor receptor (EGFR) at Thr-669 and Ser-1046/1047 via ERK and p38 pathways, respectively. In the present study, we investigated the roles of ligand-induced phosphorylation of serine and threonine residues in EGFR-overexpressing MDA-MB-468 breast cancer cells. Epidermal growth factor and heregulin, an ErbB3 ligand, induced the phosphorylation of Thr-669 and Ser-1046/1047. Inversely, constitutive tyrosine phosphorylation of the C-terminal domain, including Tyr-1068, was significantly downregulated on ligand stimulation. Inhibition of the ERK pathway by U0126 blocked ligand-induced Thr-669 phosphorylation as well as Tyr-1068 dephosphorylation. Downregulation of constitutive tyrosine phosphorylation of EGFR in HEK293 cells stably expressing the wild type was abolished by substitution of Thr-669 for Ala. In an asymmetric EGFR homodimer structure, one Thr-669 in the receiver kinase of the dimer was involved in downregulation. Similarly, Thr-669 in an EGFR-ErbB3 heterodimer also participated in tyrosine dephosphorylation. These results indicate that ERK-mediated Thr-669 phosphorylation suppresses constitutive tyrosine phosphosphorylation in the homo- and heterodimer asymmetric conformations of the EGFR.

This paper describes the health and nutritional status and quality of life (QOL) of suburban villagers in the Solomon Islands 3 y after the 1998-2003 ethnic conflict. Cross-sectional data were obtained from a small community located 50 km east of the capital city (n=206, 87 adults and 119 children). A health survey involving urine analysis, anthropometry, and blood pressure measurements was conducted to assess health and nutritional status and child growth. Simultaneously, 57 non-randomly selected adults participated in the QOL questionnaire survey. Results of anthropometry show that participants had good health and nutritional status (mean BMIs: 22.8 and 21.7 for men and women, respectively) and 73% of boys and 83% of girls were judged `normal body size' based on their BMI values. Urinalysis revealed that 88% of the participants were healthy and indicated that they consumed considerable amounts of purchased food such as rice and tinned meat. These findings suggest that the population's lifestyle had essentially recovered from the ethnic conflict. However, possible consequences of the ethnic conflict on the QOL scores were observed in the environmental domain. This study found a positive association between body fat and QOL. This could be interpreted in terms of the traditionally positive view of large bodies in the South Pacific and as resulting from unstable social conditions prevailing after the ethnic conflict.

The prevalence of obesity and its relationship to lifestyle habits was studied in Minami Furano Town, a small farming town in Hokkaido, Japan. All residents of Minami Furano Town over 18 years of age were given an anonymous, self-administered questionnaire in March, 2002. Logistic-regression analysis was performed separately for each gender with obesity as the dependent valuable, and lifestyle risk factors as explanatory variables. The prevalence of obesity (BMI > or = 25) was 30.2% in men and 29.2% in women. The risk factors for obesity included "smoking", "having no hobby", "low intake frequency of green vegetables", "high intake frequency of cooking oil" and eating "a large quantity at dinnertime" in men. For women the risk factors included "age", "stress", "drinking alcohol", "eating quickly", "low tooth brushing frequency" and "irregular health checks". Nearly one third of the adults in this town were obese when assessed using the Japanese criteria of BMI > or = 25. Few women were employed full-time and they had limited opportunity for periodic health checks. Improved community-based nutrition activities are needed for women.

Phenolic compounds, in general, exhibit antioxidant and antibacterial activities. We studied antimicrobial activity of the phenolic antioxidants, propofol (2,6-diisopropylphenol), tocopherol, eugenol, butylated hydroxyanisole (BHA), and several of their dimer compounds. Dipropofol (dimer of 2,6-diisopropylphenol) showed strong antibacterial activity against gram-positive strains including methicillin resistant Staphylococcus aureus (MRSA) and vancomycin resistant Enterococci (VRE), while propofol and other monomeric and dimeric phenols having methyl or tert-butyl groups showed no remarkable activity. The results indicated that the dimeric structure of 2,6-diisopropylphenol moiety may play an important role in the antibacterial activity.
